Dear Parent or Guardian,
Based on clinical trial results reported by Pfizer, the FDA and CDC have approved expanded eligibility for the Pfizer mRNA COVID vaccine to include children aged 5-11 years. We are providing both ages 5-11 years of age and 12 and older.
We have been working with Maine General Hospital to hold vaccine clinics at our school. We want to make the COVID-19 vaccination, with parent permission, as convenient as possible. There are several important reasons to consider taking advantage of this opportunity:
Pfizer has reported that this vaccine is safe and tremendously effective at protecting children from COVID-19 infection.
Because it is so effective, a vaccinated person is very unlikely to become a source of COVID-19 spread to anyone else. As more and more people in our community get vaccinated, we will see fewer and fewer COVID-19 cases.
Fewer cases will get us closer to returning to the lives we miss, including school without masks and social distancing.
Vaccinated people, including school students, do not need to quarantine in the event of a COVID-19 exposure.
Vaccine clinic dates will be offered on the following dates:
DATE: for dose 1: December 21, 2021
DATE: 2nd dose: January 11, 2022

Fill out the permission form by Friday, December 10, 2021. Forms can also be returned by email to firstname.lastname@example.org or faxed to your child’s school.
